Police seek witnesses to 3-car crash on Christmas Eve

ALBANY, Ore. -- Police are seeking witnesses to a three-car crash Christmas Eve that sent nine people to area hospitals.

Anyone with information about the 6:30 p.m. crash on Highway 20 between Corvallis and Albany is asked to contact Trooper Jeff Freitag at (541) 967-2026 ext. 4350.

According to Freitag, a 1991 Ford Taurus driven by Andrew Scott Tokstad, 19, of Salem, Ore., was westbound on Highway 20 near milepost 8 when, for an unknown reason, the car crossed the centerline. 

The Ford Taurus collided with the side of an eastbound 2006 Suzuki Forenza driven by Andrea L. Logan, 18. Logan and her tree passengers were not injured. 

After striking the side of the Suzuki, the Ford collided head-on with a 1999 Chrysler Cirrus.

The Ford's driver Tokstad was transported by ambulance to Albany General Hospital with minor injuries. Three passengers were also transported with minor or possible injuries. They were identified as Stevie Simpson, 20, and two children ages 15 months (female) and two months (male).

The Chrysler's driver, Sudhakar Thakillapati, 32, of Corvallis, Ore., was transported to Good Samaritan Hospital in Corvallis with serious injuries. Four passengers from Corvallis were also transported to hospitals with minor to serious injuries.  They are identified as:

Trashana Hanumantha Rao, 28, with minor injuries to Good Samaritan Hospital in Corvallis
Silvakamari Thakillapati, 31, with unknown injuries to unidentified hospital.
Sushma Thakillapati with serious injuries to Albany General Hospital, then transferred to Legacy Emanuel Hospital in Portland
Bindu Lumen with serious injuries to Albany General Hospital then transferred to Legacy Emanuel Hospital in Portland
